d\u00e9cid\u00e9 de faire appel aux experts Gerontim pour <strong>r\u00e9aliser le nouveau projet d\u2019\u00e9tablissement<\/strong>, en impliquant personnel et r\u00e9sidents.<\/p><p>D\u00e9couvrez data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Si la direction de la R\u00e9sidence Saint Martin a d\u00e9cid\u00e9 de faire appel \u00e0 Gerontim pour l\u2019accompagner dans la r\u00e9alisation de son nouveau <a href=\"https:\/\/www.gerontim.com\/fr\/prestations\/projet-etablissement-ou-service\/\">projet d\u2019\u00e9tablissement<\/a>, c\u2019\u00e9tait en partie pour <strong>impliquer davantage chaque personne actrice au sein la R\u00e9sidence<\/strong>. En effet, il leur paraissait essentiel de placer les besoins de chacun au c\u0153ur de la d\u00e9marche et de <strong>favoriser une r\u00e9flexion collective<\/strong>.<\/p><p><em>\u00ab\u00a0Nous souhaitions un accompagnement qui aille au-del\u00e0 d\u2019une simple consultation interne. L\u2019intervention de Gerontim a permis de cr\u00e9er une autre dynamique au sein de notre personnel et d\u2019inclure r\u00e9ellement nos r\u00e9sidents ainsi que leurs familles dans cette r\u00e9flexion, afin de remettre tout le monde au centre du processus.\u00a0\u00bb<\/em><\/p><p>Apr\u00e8s avoir fait passer des questionnaires qui portaient sur les attentes de chacun, la direction a organis\u00e9 plusieurs r\u00e9unions avec le CVS afin de <a href=\"https:\/\/www.gerontim.com\/fr\/articles\/experts\/preparation-projet-etablissement-service\/\">pr\u00e9parer le projet d\u2019\u00e9tablissement<\/a>. <a href=\"https:\/\/www.gerontim.com\/fr\/prestations\/auto-evaluation-qualite-esms\/\">L\u2019\u00e9valuation HAS<\/a> qui a eu lieu en janvier 2024 a \u00e9galement permis de <a href=\"https:\/\/www.gerontim.com\/fr\/articles\/experts\/ameliorer-demarche-qualite-en-ehpad\/\">d\u00e9gager des objectifs concrets et des lignes directrices pour construire ce projet d\u2019\u00e9tablissement.<\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-507a9ec elementor-section-stretched actu-texte elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"507a9ec\" data-element_type=\"section\" data-e-type=\"section\" data-settings=\"{&quot;stretch_section&quot;:&quot;section-stretched&quot;}\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-d76a938\" data-id=\"d76a938\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-6c3a900 elementor-widget elementor-widget-heading\" data-id=\"6c3a900\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Des objectifs et une vision pour le futur<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-23c4266 elementor-widget elementor-widget-heading\" data-id=\"23c4266\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Bientraitance et ouverture de l'\u00e9tablissement<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f4115e1 elementor-widget elementor-widget-text-editor\" data-id=\"f4115e1\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Le projet d\u2019\u00e9tablissement a \u00e9t\u00e9 articul\u00e9 autour de deux objectifs principaux\u00a0: la <strong>bientraitance<\/strong> et <strong>l\u2019ouverture de l\u2019\u00e9tablissement \u00ab\u00a0\u00e0\u00a0\u00bb et \u00ab\u00a0sur\u00a0\u00bb l\u2019ext\u00e9rieur<\/strong>. Ces objectifs ont \u00e9t\u00e9 d\u00e9clin\u00e9s dans\u00a0:<\/p><ul><li>Un <strong>projet de soin<\/strong> qui prend en compte la fragilit\u00e9 des r\u00e9sidents,<\/li><li>Un <strong><a href=\"https:\/\/www.gerontim.com\/fr\/articles\/experts\/role-projet-vie-animation-ehpad\/\">projet de vie et d\u2019animation<\/a><\/strong> centr\u00e9 sur l\u2019ouverture vers l\u2019ext\u00e9rieur,<\/li><li>Un <strong>projet d\u2019h\u00e9bergement<\/strong> pour maintenir son mode de vie.<\/li><\/ul><p>L\u2019id\u00e9e \u00e9tait \u00e9galement de mettre en avant les futurs <a href=\"https:\/\/www.gerontim.com\/fr\/prestations\/projets-architecturaux\/\">projets architecturaux<\/a> de l\u2019\u00e9tablissement.<\/p><p><em>\u00ab\u00a0Nous pr\u00e9voyons aussi diff\u00e9rents travaux pour <strong>am\u00e9liorer le quotidien des r\u00e9sidents<\/strong>. Par exemple, cela va se concr\u00e9tiser par du r\u00e9am\u00e9nagement de l\u2019ensemble des logements et des espaces communs pour faire un <strong><a href=\"https:\/\/www.gerontim.com\/fr\/articles\/experts\/ehpad-lieu-de-vie-par-nature\/\">lieu de vie et de partage<\/a><\/strong> o\u00f9 familles et personnes ext\u00e9rieures pourront se rencontrer. L\u2019ouverture vers l\u2019ext\u00e9rieur nous tient beaucoup \u00e0 c\u0153ur car la R\u00e9sidence profite d\u2019un parc magnifique. Nous souhaitons donc mettre \u00e0 profit ce cadre pour am\u00e9liorer le confort des r\u00e9sidents. \u00bb<\/em><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b30f25c elementor-widget elementor-widget-heading\" data-id=\"b30f25c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">L\u2019unit\u00e9 de vie prot\u00e9g\u00e9e au c\u0153ur des r\u00e9flexions<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c28661c elementor-widget elementor-widget-text-editor\" data-id=\"c28661c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>D\u2019une capacit\u00e9 de 14 places, \u00ab l\u2019Unit\u00e9 des 1000 pens\u00e9es \u00bb est une unit\u00e9 de vie prot\u00e9g\u00e9e accueillant des personnes \u00e2g\u00e9es d\u00e9sorient\u00e9es, atteintes de la maladie d\u2019Alzheimer ou de maladies apparent\u00e9es, dans le but de maintenir leur autonomie et leurs capacit\u00e9s.<\/p><p>B\u00e9n\u00e9ficiant d\u2019un projet d\u2019accompagnement personnalis\u00e9, \u00ab l\u2019Unit\u00e9 des 1000 pens\u00e9es \u00bb a \u00e9t\u00e9 au c\u0153ur des r\u00e9flexions afin de permettre aux professionnels <strong>d\u2019accompagner du mieux possible ces r\u00e9sidents<\/strong> tout en pr\u00e9servant leur identit\u00e9 et leur individualit\u00e9. La consultante s\u2019est ensuite immerg\u00e9e dans la vie de l\u2019\u00e9tablissement, afin de co-construire un projet d\u2019\u00e9tablissement qui <strong>refl\u00e8te les valeurs et les ambitions<\/strong> de la R\u00e9sidence. L\u2019intervention de Gerontim a notamment permis de\u00a0:<\/p><ul><li>Enrichir les r\u00e9flexions et d\u2019\u00e9clairer sur la d\u00e9marche pour construire un projet align\u00e9 avec leurs
