Pour permettre cela, le rendu de Gerontim s\u2019accompagne d\u2019un <strong>tableau de programmation et de suivi<\/strong> qui permet de :<\/p><ul><li>Planifier les objectifs et actions sur les 5 ans \u00e0 venir, pr\u00e9cisant pour chaque objectif les constats initiaux, les objectifs g\u00e9n\u00e9raux et op\u00e9rationnels d\u00e9finis, les actions envisag\u00e9es pour concr\u00e9tiser les objectifs, les moyens n\u00e9cessaires aux actions, les r\u00e9alisateurs des actions, l\u2019\u00e9ch\u00e9ancier et les indicateurs d\u2019\u00e9valuation<\/li><li>D\u00e9finir pour chaque \u00e9quipe et agent une feuille de route issue de la planification des objectifs et actions, et ainsi pousser la dynamique manag\u00e9riale<\/li><li>Effectuer un suivi au moins annuel des objectifs
